  • The aim of this report was to discuss the development and content of a guide on clinical performance and basic clinical skills for medical students. We published the first edition of this guide in 2010 and will publish the second edition in 2016. Initially, we took a survey on important clinical presentations and fundamental clinical and technical skills in 41 medical schools in Korea. Ultimately, we chose 80 core clinical presentations and 56 clinical skills. In the guide to basic clinical skills, we described the physical examination and technical skills according to the preprocedural preparation, procedure, and postprocedural process. In the guide on clinical performance, we reviewed patient encounters—from history taking and the physical examination to patient education. We included communication skills, principles of patient safety, and clinical reasoning schemes into the guides. In total, 43 academic faculty members helped develop the basic clinical skills guide, 75 participated in establishing the clinical performance guide, and 16 advisors from 14 medical specialty societies contributed to the guide. These guides can help medical students approach patients holistically and safely.

효과적인 진료를 위해 의사는 환자의 이야기를 들으면서 추론을 하고, 동시에 환자에게 공감하면서 질문하는 등 동시에 다양한 활동을 한다. 신체진찰을 할 때에도 환자를 배려하면서 정확하게 수행해야 하고, 진찰 소견을 해석하고 진단을 세울 수 있어야 한다. 또한 환자의 생각과 원하는 기대사항에 대해 파악하면서 필요한 정보를 설명하고 치료에 대해 논의하여야 한다. 술기 또한 복합적이다. 의사는 사전에 환자에게 술기에 대해 동의를 구하고, 정확하게 술기를 시행해야 한다. 환자가 진료에 협조하도록 해야 하는 한편, 무균 조작과 폐기물 처리 등 환자안전 수칙도 철저히 준수해야 한다. 술기가 끝난 후에는 환자에게 주의사항을 교육해야 하며, 의무기록도 작성하고, 다른 의료진에게 처치명령도 전달하여야 한다.
학생들이 진료를 잘 수행하기 위해서는 의과대학에서 학습할 때부터 이러한 역량을 통합적으로 개발해야 한다. 한국의과대학의학전문대학원협회는 학생들의 통합적 학습에 도움이 되고자 2008년부터 지침을 개발해오고 있으며 2016년에 2판을 발간할 예정이다. 지침을 사용하는 교수와 학생 입장에서는 지침이 어떻게 개발되었는지 의문을 제기할 수 있다. 또한 개발 첫해에 비해 해마다 내용이 조금씩 수정되면서 지침의 구성이 점진적으로 바뀌었기에 교수와 학생들이 교수학습에 활용하는 데 혼란을 가져올 수 있다.
이에 저자들은 지침의 타당성과 신뢰성을 확보하기 위해 어떤 개발과정을 거쳤는지 그 경과를 보고하고 2판이 1판에 비해 달라진 내용이 무엇인지에 대해 보고하고자 한다.
1. 지침 개발 진행 과정

1) 편집위원회 구성 및 방향 설정

한국의과대학•의학전문대학원협회는 2008년 8월에 협회의 교육위원와 전문위원들이 중심이 되어 지침 개발을 위한 편집위원회를 구성하였다. 당시 편집위원은 총 14명으로, 전공분야는 내과, 외과, 소아청소년과, 산부인과, 가정의학과, 마취통증의학과, 의학교육학, 의료법윤리학 등이었다. 편집위원회는 대한의학회 진료지침서[1], 한국의과대학•의학전문대학원협회의 학습목표, 의사국가시험 실기시험 문항, 캐나다 의학회 평가목표[2], 기타 국내외 참고문헌 등을 검토하였다. 검토 결과 진료지침과 술기지침을 별도로 개발하기로 하였다. 그리고 진료지침은 필수 임상표현을 중심으로 하기로 의견을 모았다.

2) 필수 임상표현과 술기 선정

편집위원회는 의과대학 졸업생이 졸업 후 의사 업무를 시작하면서 흔히 접하거나, 간과했을 경우 심각한 위해를 가할 수 있는 임상표현과 술기를 선정하고자 하였다. 이에 편집위원회는 기존의 참고문헌들에 기술된 임상표현과 술기를 모두 모은 후, 각 의과대학 및 의학전문대학원에 설문을 실시하여 각각 중요도를 평가하도록 하였고, 그 의견을 수렴하였다.

3) 개발위원 위촉 및 오리엔테이션

지침의 내용을 저술할 개발위원은 각 대학에서 추천을 받아 편집위원회가 위촉하였는데, 실기시험이나 실기교육 경험이 풍부한 교수를 원칙으로 하였다. 총 41개 의과대학 중 한 대학당 최소 1명 이상의 개발위원을 위촉하였다. 초판 발간 당시 개발위원은 기본임상술기지침의 경우 총 49명, 기본진료수행지침의 경우 총 54명이었다. 편집위원회는 집필에 동의한 교수들을 대상으로 지침 개발 원칙과 방법을 안내하는 워크숍을 2회 시행하였고 이후 구체적인 내용을 개발하는 편집워크숍을 매달 시행하였다.

4) 지침 출간 및 편집위원회 재구성

기본임상술기지침과 기본진료수행지침 초판은 2010년 3월에 출간되었다. 편집위원회는 초판이 발간된 이후 각 의과대학에 설문을 시행하여 수정 및 건의사항을 수렴하였다. 편집위원회는 초판 발행과정과 건의사항을 수렴한 결과, 완성도 있는 지침이 발간되려면, 보다 전문적인 집단이 장기적으로 편집팀으로 활동하는 것이 좋겠으며, 최소한 5년은 지침을 지속적으로 수정하고 개선해야 한다고 의견을 모았다.
이에 편집위원을 7인의 전문가 집단으로 재구성하였다. 전공 과는 내과, 외과, 소아청소년과, 산부인과, 응급의학과, 가정의학과, 신경과로 하였으며 지역별 실기시험 컨소시엄의 추천을 받아 위촉하였다.

5) 연도별 지침 수정과정

지침의 항목과 설명 내용은 매년 시행하는 설문결과를 바탕으로 추가 또는 삭제하였다. 2011년에 기본임상술기지침 초판개정1판(version 1.1)과 기본진료수행지침 별책을 발간하였다. 이때에도 지침개발워크숍을 1회 시행하였고 편집워크숍을 매달 시행하였다. 2013년에 기본임상술기지침 초판개정2판(version 1.2)과 기본진료수행지침 초판개정1판(version 1.1)을 발간하였다. 2014년에 기본임상술기지침 초판개정3판(version 1.3)과 기본진료수행지침 초판개정2판(version 1.2)를 발간하였다. 2015년에 기본임상술기지침 초판개정4판(version 1.3)과 기본진료수행지침 초판개정3판(version 1.3)을 발간하였다.

6) 제2판 발간 작업

2016년 제2판 발간을 앞두고 편집위원을 총 11인으로 확대하였고 정신건강의학과, 안과, 흉부외과, 의료커뮤니케이션 등 4개의 전공분야 전문가를 추가 위촉하였다. 또한 총 14개의 관련 전문학회로부터 자문위원 16인을 추천 받아 위촉한 후 자문위원 워크숍을 1회 시행하여 최종 수정사항에 대해 자문을 받았다. 2015년까지의 개발위원 총수는 기본임상술기지침의 경우 43명, 기본진료수행지침의 경우 75명이다.
2. 기본임상술기지침의 구성과 내용

1) 지침의 구성

각각의 술기는 시술 전 준비, 시술, 시술 후 마무리라는 3개의 큰 틀로 직무를 분류하여 기술하였다. 각각의 술기에 대해 개요와 학습성과목표, 적응증, 준비물 등을 기술하였고 절차 순서대로 작성하였다. 절차의 전체 과정을 모두 포함하였으며, 환자안전의 관점에서 중요한 의사소통, 무균조작, 시술위치, 폐기물 처리 등을 빠짐없이 포함하고자 하였다.

2) 술기의 구체적 내용

초판 발간 당시 필수 술기는 총 49개였고 2016년 2판에서 설명하는 술기는 총 56개로, 신체진찰 21개, 기본술기 35개이다(Table 1). 초판개정1판에는 시력검사, 남자 외성기 진찰, 여성 골반 및 비뇨생식기 진찰, 말초혈관 진찰, 산전 태아 안녕평가, 코위장관 삽입 등을 추가하였다. 그리고 찢긴상처드레싱과 봉합상처드레싱은 상처드레싱으로 통합하였다. 성인 기본 심폐소생법과 전기 제세동법도 통합하여 기술하였다. 기본외상처치술과 균배양검사채뇨는 삭제하였다. 초판개정2판은 술기의 내용을 주로 검토하였고 초판개정3판은 사진을 일괄적으로 재점검하였다. 초판개정4판은 성인 외상환자의 평가와 처치, 기관내흡인, 배 X-선 프레젠테이션을 추가하였고 팔부목고정은 팔다리부목고정으로 확대하였다. 또한 색인을 추가하였다. 제2판에는 관절천자, 흉수천자, 복수천자를 추가하였으며 술기 습득을 위한 교수학습전략을 부록에 첨부하였다. 초판부터 제2판까지 변화한 술기의 수는 Table 2와 같다.
3. 기본진료수행지침의 구성과 내용

1) 지침의 구성

각각의 임상표현에서 진료는 진료시작, 병력청취, 신체진찰, 환자교육의 순으로 정리하였다. 각 임상표현별로 개요와 학습성과목표, 대표 증례, 문제해결개요를 제시하였으며 관련증례를 추가하여 추론학습에 도움이 되도록 하였다. 문제해결개요는 임상표현의 원인과 병태생리를 포함한 스키마의 형태를 우선하였고, 스키마가 어려운 경우 알고리듬의 형태를 취하였다. 폐쇄형 질문 외에 사용할 수 있는 개방형 질문들의 예를 제시하였으며, 진료시작과 환자교육에서 사용하는 의사소통 기법을 포함하였다[3]. 병력청취와 신체진찰 항목들은 문제해결개요와 연결하여 그 우선순위와 목적을 함께 제시하였다(Appendix 1).

2) 구체적 내용

초판 발간 당시 주요 임상표현은 총 49개였고 2016년 2판에서는 총 80개로, 기본진료 60개, 상담관리 20개이다(Table 3). 별책에는 이상지질혈증, 황달, 소변감소, 약물복용상담, 환자상태보고, 동의서받기 등의 항목을 추가하였다.
초판개정1판부터 총론을 기본 개념과 의료진 간 의사소통으로 나누어 기술하였고 진료의 단계는 삭제하였다. 그리고 상담관리를 따로 구분하여 정리하였고, 부종, 두드러기, 유방통, 복부팽만, 생리통, 질출혈, 고혈당, 폐경, 수술전 준비, 자살 등을 추가하였다. 초판개정2판에는 급성 두통, 빨간 눈, 목 덩이, 유두 분비물, 빈뇨, 보행장애, 성폭행 등을 추가하였다. 초판개정3판에는 보채는 아이, 소아 호흡곤란, 하반신불완전마비, 수술 후 관리 등을 추가하였다. 제2판은 총론을 교수학습전략과 의사소통으로 나누어 기술하였고, 늘어지는 아이, 저신장, 약한배뇨, 손발저림, 성인 예방접종 등을 추가하였다. 초판부터 제2판까지 변화한 총론 주제와 임상표현의 수는 Table 4와 같다. 그 외 기본진료수행지침의 기존 내용은 매년 다시 검토하고 수정하는 작업을 거쳤다.
초판 이후 개정판부터 달라진 가장 중요한 변화는 각론을 장기계통별 분류에서 해부구조에 따른 분류로 변경한 점이다. 그 이유는 장기계통별로 임상표현을 배치하였을 경우 이것이 임상추론을 방해한다는 편집위원회의 판단에서였다. 예를 들어 호흡곤란을 ‘호흡기 계통’에 배치한 순간, 호흡곤란의 원인으로 폐질환을 주로 떠올리게 되는 부작용이 발생할 수 있다고 편집위원회는 생각하였다. 마찬가지로 실신을 ‘순환기 계통’에 배치하면 학생들은 그 원인으로 심장 문제를 주로 고려하게 되면서 추론의 초점이 좁아진다. 따라서 편집위원회는 임상표현을 장기계통별로 배치하지 않음으로써 학생들이 폭넓게 임상추론을 하는 것을 돕고자 하였다.
그동안 진료지침서[1]가 소수 존재하기는 하나, 학생 수준에 도움이 될 만한 통합적 지침은 미흡한 실정이었다. 따라서 이 지침은 앞으로 학생과 교육자가 술기와 임상표현별로 교수학습하는 데 많은 도움이 될 것으로 생각한다. 특히 최근 제안되고 있는 스키마를 이용한 임상추론[4]이나, 임상표현에 바탕을 둔 교수학습[5,6,7], 그리고 통합적 실습[8] 등을 실습 현장에서 구현하는 데에도 여러 모로 쓸모가 있을 것이다.
진료와 술기를 효과적으로 학습하기 위해서는 학습자가 스스로 경험해보고 그 경험을 축적해나가는 것이 중요하며 실제 상황과 관련된 환경에서 환자에게 전인적으로 접근하면서 적극적으로 의사 결정하는 과정까지 학습하는 것이 좋다[9]. 임상추론과 의사소통을 접목한 복합적인 진료나 술기를 습득하는 데는 지속적인 노력이 필요하다[10]. 따라서 이 지침을 바탕으로 조기부터 지속적이고 체계적으로 학습할 것을 제안한다.
