Medical Education in an Asian Context

Needs assessment for faculty development in dental education, at the University of Health Sciences, Lao PDR

Due to the short history of dental education in Laos, the educational system is still incomplete, with only a few faculty development (FD) programs. This study aims to identify the needs assessment for FD, in Lao dental education. A survey was conducted, with dentists from the Faculty of Dentistry, in 2022. Data on demographics, perceived importance, and ability, on the 13 roles and competencies of teachers, as well as the 26 FD items’ needs assessment were collected. Data were compared between the two groups (lecturers and assistant teachers), and analyzed to identify different needs. Sixty-seven responses were included, after excluding inappropriate ones. Lecturers and assistant teachers expressed their needs for developing a syllabus, and teaching using simulation. Lecturers revealed the need to improve their roles as resource developers, followed by learning facilitators and lecturer in a classroom setting. Assistant teachers reported their prioritized needs for improving their role as on-the-job role models, followed by lecturer in a classroom setting, and mentor, personal adviser, or tutor. Assistant teachers showed higher educational needs scores than lecturers, in most questionnaire items. Although the needs assessment of FD indicates different needs, based on the differing roles and responsibilities, assistant teachers’ needs scores are generally higher. An FD program could prioritize the most common needs of both groups at an early stage, but the topics most needed by each group should also be considered. This study can inform a future FD program, to improve Lao dental education.

Simulation-based education for selected communication skills: exploring the perception of post-graduate dental students

Purpose
The role of simulation-based education (SBE) in enhancing communication has been established in the literature. To ensure achieving desired outcomes from SBE initiative, the individual learners, their experiences, and environments need to be considered. This study aimed at exploring the perception of post-graduate dental students regarding their participation in SBE sessions, around selected communication skills, designed in alignment with the assumptions of adult learning theory and steps of Kolb’s experiential learning cycle.
Methods
This study utilized a qualitative design. Six focus-group sessions were conducted following the SBE sessions. The generated data was inductively investigated using a multi-staged participant-focused approach to thematic analysis, based upon constructivist epistemology. NVivo was utilized to facilitate text Fragments’ coding and categorization.
Results
Forty-three post-graduate dental students participated. The analysis resulted in 16 categories spread across five sequential phases of the SBE experience. The “input” theme referred to the resources needed for the SBE process, and included three categories: facilities, personnel, and teaching materials. The second theme included steps of SBE “process”: pre-brief, simulation, and debrief. The third, fourth, and fifth themes constituted the short- and longer-term results of the SBE intervention. The participants reported instant benefits on confidence in their expertise in communicating with patients and their guardians. Additionally, perceived effects on the empathy and professionalism were reported.
Conclusion
SBE, that is based upon adult and experiential learning theories, and developed after thorough consideration of the individual learners, and their experiences and learning environments, holds potential in enhancing communication skills among post-graduate dental students.

Cheating behaviors and related factors at a Korean dental school

Purpose
This study aimed to investigate students’ cheating behaviors, perceptions, and risk factors for cheating.
Methods
The author conducted a questionnaire survey of 375 undergraduate students at the Dankook University College of Dentistry in 2017. The questionnaires consisted of three parts: individual information, a moral sensitivity test, and cheating behaviors. Students rated whether they were involved in 28 cheating behaviors and their intolerant attitude for each behavior. Each student received a cheating grade of severity and diversity from 0 to 4 according to the degree of commitment in cheating behaviors. Chi-square and correlation tests were performed among cheating grades, individual factors, and moral sensitivity.
Results
Most students admitted having engaged in at least one cheating behavior (92.2%). School years, intolerant attitudes towards cheating, perceived prevalence, study time, and academic performance showed significant correlations with students’ cheating grades.
Conclusion
These findings indicated that cheating behaviors were a very serious and prevalent problem at this dental college in South Korea. This is a critical issue that must be addressed. Dental school faculty members need to work together to develop policies, ethics curriculum and to improve students’ attitudes. It is also important to encourage students to learn and devote their time to study to reduce cheating behaviors.

Purpose
The purpose of this study was to examine whether non-cognitive student attributes such as learning style and personality type affected academic performance in a flipped learning classroom of a pre-dental undergraduate science course.
Methods
‘Biodiversity and Global Environment,’ a 15-week, 3-credit course, was designed as a flipped class in Seoul National University School of Dentistry in 2017. Second-year pre-dental students were required to enroll in the course and to engage in online learning and in-class discussion. The Kolb’s Learning Style Inventory and the Myers-Briggs Type Indicator were conducted to measure non-cognitive student factors. Independent samples t-test and multivariate regression analyses were used to examine the relationships between self-rated measurements and academic achievement.
Results
More than half of the students enrolled in the flipped science course had an assimilator learning style (50%), followed by convergers (24%), accommodators (16%), and divergers (10%), and their personality types were dominated by the introverted, sensing, thinking, and judging types, respectively. Examining group differences using the t-test demonstrated a significant relationship between the diverger group and higher academic success. In particular, the multivariate regression analysis indicated that both thinking types and female students performed better in discussion than feeling types and male students.
Conclusion
To operate the flipped learning classroom more effectively in medical and dental education, the instructor should carefully develop and apply a more tailored facilitation and relevant assessment by considering student learning styles and personality types.

The dentist-scientist career pathway in Africa: opportunities and obstacles

The future of evidence-based dentistry in developing Africa heavily depends on a sustainable establishment of a vibrant dentist-scientist workforce. A dentist scientist is saddled with the responsibility of carrying out robust cutting edge research projects that are inspired by clinical experience. Currently, there are no pipelines in place to systematically train such dentists, neither are there programs in place to allow trained African dentists choose such a career pathway. A dentist-scientist is a person who studied oral, dental, maxillofacial (or craniofacial) diseases, prevention, and population sciences (obtaining a medical degrees such as bachelor of dental surgery [BDS] or BChD) alone; or in combination with other advanced degrees such as doctor of dental surgery (DDS)/doctor of philosophy (PhD) or BDS/PhD. This situation has resulted in overdependence of African clinical practice on research findings from technologically advanced Western countries and a decline in clinical research capacity building. The career path of a dentist-scientist should involve research along the spectrum of basic biomedical sciences, translational, clinical and public health sciences. There are several factors responsible for the ultra-low count of dentist-scientist in the heterogeneous African communities such as: poor biomedical research infrastructure; lack of funding; absence of structured dentist scientist career pathways; lack of personnel, inter alia. Hence, this review hopes to discuss the opportunities of setting up a dentist-scientist training pathway in Africa (as obtains in most developed world settings), identify opportunities and prospects of developing an African dentist-scientist workforce, and finally discuss the challenges involved.

Professional and personal enhancement: a pragmatic approach in dental education

Purpose
Students of health education are often offended by the transitions and challenges they face while encountering diverse people, ideas and academic workloads. They may be offended because of reasons not only related to their societal background but also to their basic competence in managing transitions. In the Asian scenario, students enter the first year of professional education in their late teen age along with the definition of self which was created by their parents. There are different issues that arise in this age group that may positively shape or negatively affect the personalities of students. They need to achieve a sense of balance between personal and professional traits on their own. Several students are often unable to cultivate the expected required qualities, which leads to an abject state of mind and hinder their progress. We identified the most common personal and professional hurdles in the lives of dental students and we provided experiential solutions to overcome the hurdles by using a sociable approach through an integrated, continuing education program.
Methods
Designing and implementing a cohesive, amalgamated and inspiring personal and professional enhancement action program for dental students.
Results
Feedback from students reflected that the needs and expectations of students vary with academic phase. In addition students expressed that this program series inculcated some positive skills, and overall, they are satisfied with the utility of the program.
Conclusion
Personal and professional enhancement of students in accordance with individual needs as well as with expected requirements needs a committed administrative action plan. Our results in this context are encouraging and can be considered for application in dental institutions.

Impacts of Problem-Based Professionalism Course in Dental Education

PURPOSE
This study explores the effects of a professionalism course based on problem-based learning (PBL) for 2nd year dental students on professional role concept and on ethical reasoning. It also investigates students' attitudes toward professionalism development program.
METHODS
The experimental group (n=36) participated in the 5-week professionalism course, which uses PBL approach while the control group (n=89) did not receive the professionalism instruction during the same period. The professionalism development program was implemented via discussions among students in small-group tutorial. Four professors, who served as group facilitators participated in faculty seminars before every tutorial. After each tutorial, students had to write essays on the cases they discussed in their groups. Both groups completed Professional Role Orientation Inventory and Defining Issue Test (KDIT) prior to and post intervention period. The experimental group also responded to questionnaires about the value of professionalism program.
RESULTS
Analysis of covariance indicated that the experimental group's responsibility score significantly improved (F=32.552, p<0.001) while their perception of agency decreased (F=29.510, p<0.001). The program had little influence on ethical reasoning measured by DIT. Twenty-eight students (78%) responded that they learned the value of other opinions. Seventy percent of the students responded that the course is worthwhile or very worthwhile.
CONCLUSION
The result reveals that students' professional role concept is associated with the learning experience in professionalism program based on PBL. Opportunities to think of professionalism in clinical situations improved students' sense of responsibility as dentist.

Perceived Stressors among Dental Students: a Scale Development

PURPOSE
The purpose of this study was to investigate the perceived sources of stress among dental students. The relationships of these stresses to the year of study and gender were also examined. METHODS: The responses from the first to fourth year dental students to 30 items adapted from Dental Environment Stress (DES) questionnaire were subjected to confirmatory factor analysis. The Likert scale, ranging from 0 (not applicable) to 4 (very stressful), was applied to these items. A total of 341 students (male=196, female=145) participated, their average age being 24.7. Multivariate analysis of variance (MANOVA) was conducted to analyze the effects of the year of study and gender on stressors. RESULTS: Confirmatory factor analysis established a five-factor model including 1) clinical practice, 2) academic load and pressure, 3) personal problems, 4) low self-esteem, and 5) school administration or climate. Subscales for each factor show good internal consistency with Cronbach's alpha ranging from .71 to .88. Mean score for factor II (academic load and pressure) was the highest among all factors for all of 4 years, which meant that primary stressors were amount of classwork, shortage of time, and competition among classmates regardless of the year of study. MANOVA result showed that the amount of stress from clinical practice and school climate generally increased through the years (p < .01), and that female students were more stressed than male students (p < .01).
CONCLUSION
Students' stress is related to the features of the curricula and the learning environment. In reducing this stress, it would be helpful to modify the curriculum as well as to introduce mentor or counselor programs.
